Neuropathy — a condition characterized by damage or dysfunction of the peripheral nerves — affects an estimated 7–10% of the global population. For those living with neuropathic pain, the symptoms can be relentless: burning sensations, tingling, numbness, shooting or electric-shock-like pain, and extreme sensitivity to touch that makes even simple activities like wearing shoes or sleeping under a blanket feel unbearable.
Traditional treatments for neuropathy often include anticonvulsants (gabapentin, pregabalin), antidepressants (duloxetine, amitriptyline), and opioid medications — all of which come with significant side effect profiles and limited efficacy for many patients. Against this backdrop, a growing body of research has positioned topical cannabidiol (CBD) as a promising natural alternative that may address neuropathic pain through multiple biological pathways.

Peripheral neuropathy refers to damage to the nerves outside the brain and spinal cord. These peripheral nerves carry signals between the central nervous system and the rest of the body, and when they are damaged, pain signals become aberrant — firing spontaneously, amplifying normal sensations into painful ones, and creating phantom sensations that have no external cause.
At the cellular level, neuropathic pain involves a complex cascade of events: neuroinflammation driven by activated microglia and astrocytes, upregulation of voltage-gated sodium and calcium channels, central sensitization of pain pathways, and the release of pro-inflammatory cytokines (TNF-α, IL-1β, IL-6) that perpetuate nerve damage and pain signaling. Understanding these mechanisms is critical because they reveal multiple potential intervention points where CBD and terpenes may exert therapeutic effects.
One of the most significant studies on topical CBD for neuropathy was published in Current Pharmaceutical Biotechnology. This randomized, placebo-controlled trial examined the effectiveness of topical CBD oil in symptomatic relief of peripheral neuropathy of the lower extremities. Over four weeks, participants who applied topical CBD experienced statistically significant reductions in intense pain, sharp pain, cold sensations, and itchy sensations compared to the placebo group. Importantly, the transdermal application was well tolerated with no adverse effects reported.
A Phase III clinical trial conducted in Thailand evaluated the efficacy and safety of transdermal medical cannabis (THC:CBD:CBN formula) for painful diabetic peripheral neuropathy. This rigorous double-blind, placebo-controlled study demonstrated meaningful improvements in neuropathic pain scores in the treatment group, supporting the case for transdermal cannabinoid delivery in neuropathy management.
Additionally, a case series published in Integrative Cancer Therapies documented that topical cannabinoids provided relief for chemotherapy-induced neuropathy patients, an especially difficult-to-treat form of neuropathic pain. The authors noted potential improvements in pain, tingling, and burning sensations with topical application.
A 2021 preclinical study also demonstrated that CBD effectively reversed mechanical and thermal allodynia, hyperalgesia, and anxious behaviors in a neuropathic pain model, with effects mediated through CB1 and TRPV1 receptors.
Neuropathic pain of the extremities — particularly in the feet, hands, and legs — may be suited for topical CBD treatment for several reasons. The peripheral nerves that are damaged in neuropathy lie close to the skin surface, within reach of transdermally delivered CBD. Topical application concentrates the active compound at the site of nerve damage, rather than distributing it systemically. Many neuropathy patients are already taking multiple oral medications, and adding a topical CBD product avoids additional systemic drug interactions. The skin’s own endocannabinoid system (with CB1 and CB2 receptors expressed in cutaneous nerve fibers) provides a direct pathway for CBD to modulate peripheral pain signaling.

He completed a general surgery residency at the University of Iowa, followed by a plastic and reconstructive surgery residency at the University of Virginia.
